Family Practice and Counseling Network (FPCN) opened in 1992 with one small health center within the Abbottsford public housing development and has since grown and developed in response to need and demand by the communities served. FPCN is now comprised of four health centers in Philadelphia that serve more than 22,000 patients annually. A program of Resources for Human Development (RHD), FPCN is a federally qualified health center network that uses a nurse-managed, integrated model to deliver primary care, behavioral health, dental, and preventive care services.
